Lens surgery in Marfan's syndrome.

چکیده

Management of the subluxed lens in Marfan’s syndrome presents one of the most challenging situations in contemporary lens surgery. Conventional approaches have included intracapsular lens extraction or a pars plana lensectomy with vitrectomy. Once the lens was removed, aphakia was corrected by glasses, contact lenses or an iris or angle supported anterior chamber IOL or a scleral sutured posterior chamber IOL. Marfan’s syndrome patients are known to have an increased risk of glaucoma as well as retinal detachment (upto 11 % of patients with Marfan syndrome, and 8–38 % in those who have dislocation of the lens or have undergone lens surgery) . Accordingly, anterior chamber implants are best avoided in this setting and a surgical approach which preserves the capsular bag and avoids disruption of the vitreous, will, at least potentially, reduce the risk of retinal detachment subsequently.

Monozygotic twins with Marfan's syndrome and ascending aortic aneurysm.

Marfan's syndrome is a hereditary connective tissue disease, in which cardiovascular abnormalities (especially aortic root dilatation) are the most important cause of morbidity and mortality. In this report, we describe two 24-year-old twins, with a history of surgery for lens subluxation and severe cardiovascular manifestations secondary to Marfan's syndrome. One of the twins suffered a type A...

Bilateral Implantation of Scleral-Fixated Cionni Endocapsular Rings and Toric Intraocular Lenses in a Pediatric Patient with Marfan's Syndrome

The management of ectopia lentis in Marfan's syndrome is challenging. Multiple disease-associated factors conspire to deprive these patients of adequate vision. While optical correction with glasses and contact lenses is usually advocated early on, the irregular astigmatism and even partial aphakia that accompanies advanced cases generally warrant surgical intervention. Several surgical strateg...

Bilateral dislocation of lens under complete voluntary control in Marfan's syndrome with cardiovascular anomaly.

THE syndrome originally described by Marfan (1896) under the name "dolichostenomalia" was subsequently called "Marfan's syndrome", but Achard (1902) suggested the term "arachnodactyly" because of the spiderlike appearance of the extended fingers. Since the orginal description of Marfan, well over 300 cases have been recorded in the literature. The condition is characterized by abnormal height, ...

Marfan Syndrome in an Iranian Family: A Case Series

Marfan syndrome (MFS) is a genetic disorder which is inherited by autosomal dominant traits. In MFS, lens displacement and cardiovascular involvement are important causes of morbidity and mortality in the clinical course of the disease. In this case study, the ocular involvement in a family with severe penetration of MFS is reported. Twelve members of a family (father, two daughters, three sons...

Spinal and pelvic parameters in Marfan's syndrome and their relevance to surgical planning.

Spinal deformities are a common feature of Marfan's syndrome and can be a significant cause of morbidity. The morphology of the scoliosis associated with this condition was previously described by Sponseller, but no correlation with the pelvic parameters has been seen. We performed a retrospective radiological study of 58 patients with scoliosis, secondary to Marfan's syndrome and related the f...
